Stone Laboratory - Student Recommendation Form 2026

All high school students must have this form completed on their behalf. In addition, any student applying for a Stone Laboratory scholarship—including high school, undergraduate, and graduate students—must also have this form completed.

Individuals completing these forms should be non-relatives who can speak to the student's academic success and maturity or work ethic (examples may include a teacher, guidance counselor, etc.). Recommendation forms should include the author’s business email.

At the end of this form, the recommender should see the message stating, “Success! Your submission has been saved!” and receive an automated confirmation email. If you did not receive this, please contact ohioseagrant@osu.edu.
